Level 3 reflects sustained leadership in quality, health equity and population health.

The Minnesota Department of Health has recertified Riverwood Healthcare Center as a Level 3 Health Care Home, the highest level of certification. Recertification confirms that Riverwood’s Aitkin, Garrison, and McGregor clinics continued to meet advanced standards for patient-centered primary care and demonstrate sustained leadership in improving health outcomes for individuals and communities.

Health Care Homes use a coordinated, team-based approach to provide comprehensive care, support prevention and chronic disease management, and partner with specialists, community organizations, and public health agencies.

Level 3 recertification reflects the clinic’s ongoing commitment to continuous quality improvement, reducing health disparities, and using population health data to guide care. Clinics at this level actively participate in community health improvement efforts and work collaboratively to address the needs of the populations they serve.

“This recertification highlights the dedication of our providers, care teams, and staff across our clinics,” said Dan Schletty, Chief Ambulatory Officer. “We’re proud to be recognized at the highest level, but what matters most is what it means for the people and communities we serve: better access, stronger partnerships, and support that helps patients achieve their best possible health.”

“Congratulations to Riverwood Healthcare Center. Sustaining a Health Care Home signifies a primary care clinic’s dedication to support patients and their families on their health care journey and provide comprehensive care for every patient,” said David Kurtzon, MDH Health Care Homes Director. “The systems that Riverwood has built and continue to strengthen extend that commitment beyond individual patients to the health and well-being of the entire community.”
