Riverwood Connects

Riverwood Connects is a program linking our community to local resources to include Food RX program and Find Help.

Your Health & Wellness Journey Starts Here
Riverwood Healthcare Center is dedicated to providing our patients with exceptional care in the hospital, clinic, surgery, and pharmacy setting, and within our community. Our facility strives to focus on our communities’ social determinants of health or the driving factors of the health and wellbeing of an individual.
What are Social Determinants of Health? Social determinants of health are the conditions in the environment where people are born, live, work, play, worship, and age that affect a wide range of health, functioning, and quality of life outcomes and risks.

Minnesota Farmers’ Market Food Hubs

In Minnesota, the Farmers’ Market Food Hub movement is connecting local farmers with communities. Farmers’ markets serve as vital hubs, bridging the gap between producers and consumers. Despite challenges, initiatives like Farmers’ Market Produce Boxes and VeggieRX are promoting local food access. Supported by USDA grants from 2022 to 2024, this project aims to build a sustainable future. Support local farmers, embrace the Food Hub, and cultivate a healthier Minnesota.
