Community Health Needs Assessment

Riverwood Healthcare Center is committed to providing high-quality, personalized and compassionate care to the communities we serve.  Every three years, Riverwood conducts a Community Health Needs Assessment (CHNA) to better understand the communities we serve by identifying, analyzing and prioritizing community health needs. In collaboration with community partners, organizations and individuals along with statistics gathered from the state priority areas are identified and strategies are put in place to address the local health needs.

Riverwood is required to complete the CHNA every three years with input from the broader community. The CHNA is a federal requirement for private, non-profit hospitals. Part of the Affordable Care Act (ACA), the CHNA is criteria for hospitals to maintain their tax-exempt status.

Riverwood Awards and Recognition

Press Ganey 2020 Guardian of Excellence Award Winner – Nationally Recognized for Emergency Care

For five years—2011, 2012, 2013, 2017 and 2018—Riverwood has been named one of the Top 100 Critical Access Hospitals (CAHs) in the country in the Chartis Center for Rural Health Health Analytics’ Hospital Strength Index. The results recognize the top critical access hospitals that form the safety net to rural communities across America – measuring them across 60 different performance metrics, including quality, outcomes, patient perspective, affordability and efficiency. Drawing on the Top 100 list, Riverwood was recognized as a Top 20 Critical Access Hospital from 1,346 peer hospitals nationwide and 78 in Minnesota for two consecutive years, 2017 and 2018. The National Rural Health Association announces the winners and recognizes each with an award at its annual conference.

In 2012, for the 5th year in a row, HealthGrades has given Riverwood a 5-star rating for excellent total hip replacement outcomes. This means patients who have joint replacements at our hospital experience few or no complications. HealthGrades’ report found that patients treated at 5-star rated hospitals experience a 73% lower risk of mortality and a 63% lower risk of complications.
