Riverwood Healthcare hires population health program manager

Rachel Johnson, Population Health Program Manager

Riverwood Healthcare Center is pleased to introduce Population Health Program Manager Rachel Johnson.

Johnson brings to Riverwood a rich history of community outreach and program management. She has obtained her master’s in business administration with an emphasis on rural health care with the College of St. Scholastica, a bachelor’s in business administration with a minor in human resources from Waldorf College, and an associate degree in practical nursing from Central Lakes College.

Johnson has experience in health care administration, program management, and value-based programming. She has worked in the areas of behavioral health, ambulatory care, community outreach, and administration.

Johnson comments, “I believe in order to provide the best care to our patients we need to build strong teams. This is done through our people all moving in the same direction with the same mission to improve the health of those we serve.”

Riverwood’s Director of Ambulatory Services Dan Schletty remarks, “Rachel will have a significant role in care integration and community engagement. Her background in health care administration, program management, and community outreach is a great addition to our Riverwood team.”

Johnson is a military spouse; her husband, Chris, serves in the Minnesota National Guard and works at Camp Ripley. They have two children and are excited to be welcoming their first grandchild this summer. In her spare time, she enjoys spending time with family, friends and her golden retriever, Auggie, as well as hiking, reading and traveling.
